New Patient registration procedure

1. Click the sign in link on this page or from the drop down menu.

2. Click on either the ‘Quick Sign Up’ or ‘Enrollment’ tabs and fill out the required information to create a user name and password.

3. Select “New Patient Visit,” or “Exercise/Nutritional Consult,” (See the services page for more information).

4. Continue following the instructions for scheduling an appointment.

5. You will receive email notification regarding your registration and upcoming appointment.

6. Read the De Novo Pittsburgh Practice Policies.

7. Print off and sign the Signature Page.

8. Bring the Signature Page with you to your appointment.

Email

Email communication is encouraged. Dr. Klueber regularly replies to email within an hour. It is appropriate to use your regular email address, but information passed through most email is unsecured and not encrypted. Sensitive information should be conveyed to your doctor by phone or during an office visit.

Phone Calls
Please call the office at anytime with questions or anything regarding your care. If you reach the voice mail please leave a message, as the doctor at De Novo Pittsburgh regularly replies to phone messages within the hour. An email reply will be sent when appropriate. All scheduling is made through our online appointment scheduler, but we are more than happy to walk you through the steps if you are having trouble.
